A comment line and a blank line take no part in column alignment. The formatter formats the rows around them as one table. It then puts the kept lines back at their original positions, byte for byte.
A comment above the header is the position to get right. The header is the first line that is neither a comment nor a blank. The formatter passes over the comment to find the header, and puts the comment back afterwards.
